Glad to hear you want to join LUGM.

You may scan the signed form and send it to LUGM's secretary Selven at this

Else you may just drop by in one of our LUGM meetups on Saturdays & leave
the form with me. The meetup happens every Saturday around 13h00 in
Bagatelle Mall. The place is usually Mugg & Bean. If there is a change in
location you'll surely hear the sound of drums on facebook :)
